CHI Memorial Medical Group has announced that Dr. Austin Hazlewood, DO, has joined CHI Memorial Family Practice Associates in Cleveland, Tennessee. Dr. Hazlewood completed his medical degree at Debusk College of Osteopathic Medicine in Harrogate, Tennessee, and finished a family medicine residency at the University of Louisville in Glasgow, Kentucky.
Dr. Hazlewood will work alongside Jamie Bird, FNP-C, and Chandler Chastain, FNP-BC. The practice provides care for patients from childhood through adulthood and focuses on disease prevention as well as overall wellness. According to the announcement, “Dr. Hazlewood and Chandler Chastain welcome new patients.”
The clinic is located at 2620 Westside Dr. NW in Cleveland and operates Monday, Tuesday, and Thursday from 8 a.m. to 5 p.m., Wednesday from 7 a.m. to 4 p.m., and Friday from 8 a.m. to noon.
